  • Affordable Care Act: Health Insurance Marketplace
    The 2017 Open Enrollment Period will begin November 1, 2016 through January 31, 2017, but you may qualify for a Special Enrollment Period if you recently lost coverage, moved, or had a change in income, household size, or citizenship status. We will provide one-on-one assistance to explore all your coverage options. Visit or call for an appointment.
  • PCN
    Primary Care Network (PCN) continues open enrollment for all qualifying adults between the age of 19 and 64 who have dependent children living in the home.  PCN is a health plan that covers basic primary services like visits to primary care providers, limited prescriptions per month (4), dental exams, etc. To be eligible you must be uninsured, age 19-64, have a dependent child under the age of 19 living in the home, and meet specific income guidelines.  If you have any questions or need additional information on eligibility, please contact Jordan School District Health Services Department 801-567-8515 or visit the State website at
